ALLERGIES / HEALTH PRECAUTIONS

Does this student have any known allergies or health precautions we should be aware of?  Please explain:

TRANSPORTATION

I further acknowledge that transportation may be provided at the discretion of SEC in such form as is approved by SEC and/or Camp Wilderness leadership.

ACTIVITY RISKS

I acknowledge that my child's participation in activities and games entail known and unknown risks that could result in physical or emotional injury, paralysis, death, or damage to my child, to property, or to third parties. The risks include, among other things, certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. I agree and promise to accept and assume all of the risks existing in this activity. My child's participation in this activity is purely voluntary, and I elect to allow them to participate in spite of the risks. Activities the child may participate in are available upon request.

EMERGENCY

In the event of an emergency and medical treatment is required, I give permission to the church staff and/or person in charge to obtain servcies for treatment as deemed necessary.

INDEMNIFY AND HOLD HARMLESS

I hereby voluntarily release, forever discharge, and agree to defend, indemnify and hold harmless Salem Evangelical Church from any and all claims, demands, or causes of action, which are in any way connected with my or my child's participation in activities, facilities and equipment, including any such claims which allege negligent acts or omissions of SEC.

PHOTO RELEASE

SEC may be taking pictures to update the church website and other promotional/informational media.  Names and personal information will not be posted unless approved by the parent/guardian.  Photos are kept secure and not shared.  By granting permission, you agree to allow your child's image to be part of ministry related public display and/or promotion without individual personal identification.

By providing my full name, I agree that it will be the electronic representation of my signature for all purposes when I use it on documents, including legally binding contracts - just the same as  pen-and-paper signature.
